This is a form to apply for, manage or cancel your direct debit arrangement for rates payments.

Complete this form to:

• set up a direct debit arrangement
• cancel your direct debit arrangement
• update your account details
• switch to a different direct debit payment option

Please note: We can only debit from a cheque or savings account, we are unable to accept credit card details.
